Trainers are very useful. Level 20 coaches give you an extra 20 training points each week for players being trained in their area of focus (offense/defense/etc). That's twice the weekly baseline, which is 10. Upgrade to level 20 when you can afford it, in every area except kicking (only benefits a single player, who you don't want to train past 12-13 skills anyway, at a cost of an extra 20K/week). Try to upgrade all coaches at the same time, because there's a signing bonus each time you sign a new staff.


Medics are also useful, because they reduce injury recovery times significantly. With that said, the cost-benefit analysis is less clear-cut. Let's say you get an average of 5 injuries per season, for an average of 2 weeks per injury. Level 20 doctors will reduce the time your players are injured by more than 50%, thus preventing the loss of 5 player-weeks' worth of training. They also reduce the chances of players being injured at all by 50%. Basically, your starters will be back on the pitch and your backups will be back in the training hall in less than half the time they would with level 1 doctors. That costs you 45K/week, or 855K/season.

My view is that, ultimately, the cost will be worth it if they prevent a single injury to your starting QB the day before you play a big game. Think of medics as an insurance policy: if you simulated 100 seasons, or 1000, the average benefits wouldn't be worth the cost. But you only get to play 2.5 seasons a year, and hugely reducing the chances of an injury dooming one of those seasons has considerable value.
