What determines a player's salary?

Is it based on the overall skills, experience, rating, talent?

I see some decent players with lower salaries and some expensive guys without much value.

So as a player improves his skills, does his salary go up each time there is improvement or are salaries adjusted at the beginning of the season? Or how does salary updates work?
