Does Attacking Direction only decide where the QB passes or does this also decided where the running backs run to? If I say Left Flank then the QB passes to the Left but the running backs still run where ever?

no. mainly it means the direction of the rushing attack.
both run and pass though going the other way happens occasionally

yep... i have noticed that on pass plays when you choose a direction, it seems more likely to play away from that is the opposition secondary is better...
