If you've arranged a trade with a team, how do you actually do it? That is, how do you propose a trade of that player to another team so he can confirm it?

SO far, I can only find the open-market player release / sell facility and no inter-team trading facility.

'Cause there's no possibilitiy of doing that, now. Players can only be sold on the transfer market, so everybody can bet on him.

One solution, provided both guys have sufficient funds, is for Coach A to transfer-list the player for way over his market value, while Coach B does similar. Then they each buy the other's player for (say) $1M.

Dangerous strategy, as it could be flagged as out of market norms, which is illegal. Also, you would lose the transfer fees.
